Your First Visit

On your first visit, you will be asked to complete paperwork including an extensive health history form. It’s very important that our doctors find out as much information about your current and previous health conditions to make an accurate assessment. Please bring any pertinent information with you such as MRI reports or X-rays. If you are currently taking any medications, please bring a complete list.

Once your paperwork has been completed, you can expect to have a conversation with one of our staff members who will review your paperwork and answer any questions you may have. You will then meet with the doctor and discuss your condition and what is bothering you.

The doctor will talk to you about what has been found and recommendations for treatment. If you are a candidate for chiropractic care, your first adjustment will be conducted at this visit.

If you are not a candidate for chiropractic care, the doctor will recommend to you some of the top doctors in our area who specialize in the care you need.

Payment and Insurance Information

Dr. Porter accepts multiple insurance plans. Please call the office to verify coverage. Our staff will gladly assist you.

If the doctor participates with your insurance plan, payment will be due at the time of treatment for any non-covered services, deductibles or co-pays. For patients’ convenience, we promptly bill all insurance carriers weekly.

If you are covered by an insurance plan that Dr. Porter does not participate with then payment in full is due at the time of treatment. Our office will complete all insurance billing at no charge and all possible reimbursements will be sent directly from the insurance carrier to you.

When possible we will call to verify the benefits of your insurance for you. However, the benefits quoted to us by your insurance carrier are not a guarantee of payment.
